Home Manager

Position OverviewThe Home Manager ensures that the daily operations for the provision of services is executed efficiently and effectively. Responsibilities include:

  • Managing the operations at assigned service locations, or settings by providing supervision and management of assigned Support Associates with responsibility for ensuring appropriate service delivery in accordance with the Individual Program Plan (IPP), as well as the agency operational budgets (i.e. managing overtime, mileage expenses, etc.);
  • Establishing systems to ensure the effective communication of both staff and client needs, including but not limited to, establishing staff schedules, communication logs, and inventory tracking tools;
  • Routinely providing direct services to the individuals residing in assigned settings, particularly for medical appointments and the coordination of medical services;
  • Working collaboratively with the Program Coordinator and nursing staff to understand and communicate changing needs or issues of the individuals living in the assigned settings;
  • Reviewing employee documentation, such as timecards and program documentation, for accuracy and completeness, with responsibility for ensuring all documentation is delivered to the regional office in accordance with prescribed timelines;
  • Ensuring the maintenance and upkeep of each service setting and notifying the Program Coordinator and management team of any environmental issues that could potentially impact the health and safety of the individuals living or working in the setting;
  • Participating in the selection of new employees;
  • Providing on the job training and mentoring for new employees;
  • Providing support for operational emergencies as needed.

Position RequirementsPosition Requirements: High School diploma or GED.  One (1) year of management experience, including the supervision of multiple employees, or five (5) years of demonstrated leadership in the provision of services to individuals with disabilities or special needs.  One (1) year of working with individuals with physical, mental, or developmental disabilities preferred.  Strong verbal and written communication skills.  Basic computer skills.  Capacity for independent decision making.  Ability to work a flexible schedule.  Ability to provide on-call support as needed.  Current valid driver’s license and compliance with Ohio liability insurance coverage standards.  Ability to maintain medication, CPR, and First Aid certification.

SUPPORT ASSOCIATE / DSP

Position Overview:  The Support Associate (Direct Support Professional – DSP) is responsible for providing supervision, support and assistance to consumers (individuals) as prescribed within the Individual Service Plan/Individual Program Plan to increase independence and quality of life.

Position RequirementsPrevious experience working with individuals with developmental disabilities is preferred, but not required. A High School diploma or GED is preferred. Current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Ability to maintain CPR and First Aid certification.

DAY SUPPORT ASSOCIATE

Position Overview:  The Day Support Associate (Direct Support Professional – DSP) is responsible for providing supervision, support and assistance to consumers participating in our Adult Day Services Program. The Day Support Associate ensures the activities offered at the program are consistent with the individual’s preferences, as stated in the ISP.

Position RequirementsPrevious experience with supporting individuals with developmental disabilities is preferred, but not required. A High School diploma or GED is preferred. Current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Ability to maintain CPR and First Aid certification.

ADULT DAY SUPPORT (ADS) PROGRAM COORDINATOR

Position OverviewThe Adult Day Support Program Coordinator ensures that the appropriate, high-quality services and supports are provided to the individuals served through effective care coordination and advocacy.

Responsibilities include:

  • Coordinating and monitoring the development and implementation of the Individual Program Plan (IPP) in compliance with company policy and procedure, as well as with external regulations;
  • Orienting and training Day Support Associates on active treatment and IPP implementation;
  • Communicating and maintaining positive relationships with families, guardians, advocates and all other members of the Interdisciplinary Team (IDT);
  • Monitoring all relevant systems, including required documentation, medical issues, finances and physical environment;
  • Resolving any discrepancies or conflicts between programmatic, medical, dietary, and vocational elements of care; Routinely reviewing program data and progress;
  • Initiating changes in the IPP collaboratively with the IDT;
  • Ensuring follow-up to recommendations for services, equipment or programs; Ensuring adequate environmental supports and assistive devices are present;
  • Coordinating the reporting and review of Unusual Incidents and Major Unusual Incidents with the ADS Manager;
  • Coordinating all intake and discharge activities;
  • Providing direct support to persons served as needed.

Position Requirements: A bachelor’s degree in social work, psychology, education, or a related field, Registered Nurse, or Medical Doctor. One (1) year experience providing direct supports to individuals with developmental disabilities. Strong verbal and written communication skills. Capacity for independent decision making. Ability to work a flexible schedule. Current valid driver’s license. Ability to maintain medication, CPR, and First Aid certification.
