At Team RMS, we frequently receive inquiries about the various services and supports we provide for individuals with intellectual and developmental disabilities. Our person-centered services, available through a home and community-based services waiver, are designed to address each individual’s unique needs.
It is important to note that all waiver services must be deemed medically necessary. When selecting a direct support professional, individuals with developmental disabilities and their support teams have the opportunity to use the free choice of provider process.
Remote Support:
This newer service allows an off-site direct service provider (Remote Support Professional) to monitor and respond to a person’s health, safety, and other needs using live communication, while offering the person more independence in their home. Remote support services in the state of Ohio are available for individuals using Individual Options, Level One and SELF waivers.
Remote Support uses Assistive Technology and two-way communication in real time, so a person can communicate with their providers when they need them. This allows RMS staff to provide support services from a distance.
When authorizing services for individuals with disabilities, Remote Support should be considered as the primary option before approving on-site Homemaker/Personal Care staff. The person’s service and support administrator (SSA) will evaluate whether Remote Support is necessary.
If necessary, remote support services are incorporated into the individual’s service plan. This plan should clearly outline backup support contact details and instructions for the person in case they wish to disable the Remote Support equipment.
In the state of Ohio, Remote Support services are provided by agency providers who are DODD-certified and have a Medicaid provider agreement. These services can be offered on a one-on-one basis or as a group service and can involve either paid or unpaid backup support.
Supported Living Services:
At Team RMS, our Supported Living services are provided by our RMS of Ohio at various locations across the state of Ohio. The services are uniquely designed to help individuals live as independently as possible to achieve a high quality of life.
Supported Living services refer to a broad category of services designed to support individuals in living as independently as possible, often in their own homes or in small, community-based residential settings.
Purpose: The purpose of these services include not only personal care and assistance with ADLs (Activities of Daily Living) but also support with skills development, community integration, and sometimes even managing financial or health-related tasks.
The scope of Supported Living typically emphasizes helping individuals maintain their own homes (rented or owned) and live as independently as possible. This might include finding housing, paying rent, and coordinating other services like transportation.
Supported Living services are be funded through a variety of sources, including state funds, Medicaid, or other programs, depending on the specific services provided and the individual’s needs.
These services are designed to assist individuals to live as independently as possible to reach personal goals and achieve a high quality of life. Our services are person-centered, supporting each individual’s strengths and abilities.
Services include: assistance with daily living skills, medication administration, housekeeping, meal preparation, personal hygiene, assistance with medical and dental appointments, assistance with managing and budgeting personal finances, coordination of transportation services, socialization opportunities, and community outings.
Supported Living services are available 7 days a week, 24 hours a day.
Adult Day:
The Team RMS locations provide social, recreational and habilitation services in a nurturing environment, with opportunities for frequent community access. Adult Day Support services are available to individuals using Individual Options, Level One and SELF waivers, Adult Day Support (also known as Day Hab or Adult Day Array) is designed to help individuals build meaningful connections within their communities.
Through this service, participants can learn:
- How to meet and interact with new people,
- What activities are available in their community,
- How to travel independently,
- And what job opportunities exist in their area. Additionally, while engaging in Adult Day Support services, individuals can receive assistance with personal care needs such as using the restroom, eating, and taking medication.
Adult Day Support services may be offered based on each individual’s unique needs, identified during an assessment. The primary goal of this service is to discover and develop each person’s skills, interests, and talents, while finding ways to foster community connections based on these strengths.
The desired outcomes of Adult Day Support should focus on building relationships with both individuals with and without disabilities and making progress toward identifying where and how each person can pursue their interests and talents alongside others. Additional support activities associated with this service may include personal care, supervision, skill reinforcement, and training in self-determination.
Agency providers who are DODD-certified and have a Medicaid provider agreement are eligible to offer this service. Adult Day Support is delivered as a group service, with staffing ratios being an integral part of the person-centered planning process. The team must carefully consider the level of supervision and support needed by the individual in different settings to ensure their health, safety, and achievement of the goals outlined in their individual service plan (ISP). A maximum of 16 individuals may participate in a group, regardless of the funding source or service delivery requirements.
ICF/IID Homes:
Team RMS offers ICF services through RMS, Inc. Our ICFs (Intermediate care facilities) are for individuals requiring a more significant level of care.
Intermediate Care Facilities (ICFs) play a vital role in Ohio’s developmental disabilities service network, offering a comprehensive range of services along with 24/7 access to emergency care.
In Ohio, ICFs have some common characteristics:
- Each facility houses at least four residents.
- ICFs maintain on-site staff around the clock, every day of the week.
- There is always someone available to assist in emergencies.
- While the appearance of ICFs may vary, it is common for residents to share a bedroom.
If you are considering moving into an ICF, it can be helpful to visit several to find one that suits you. Visiting at different times of the day allows you to see daily activities and ask questions to better understand the environment.
For those interested in moving into an ICF with eight or more residents, a meeting with your Service and Support Administrator (SSA) from your local county board of developmental disabilities is required for Pre-Admission Counseling.
Pre-Admission Counseling is an important step before transitioning to an ICF. During this meeting, your SSA will ensure you are fully informed about the support options available to you, whether you choose to live in an ICF or another setting. If you decide to move into an ICF after this counseling, the ICF staff will reach out to discuss the next steps.
Once the decision to move into an ICF is made, individuals and their families will begin working with the ICF’s staff, known as Qualified Intellectual Disabilities Professionals (QIDPs). The QIDP will engage with you, your family, and other supporters to discuss your strengths, goals, and areas where you might need assistance from the ICF staff.
Using this information, the QIDP will collaborate with you to create a service plan outlining the services you will receive while residing in the ICF. You will meet with your QIDP at least once a year to ensure the services provided align with your needs. Additionally, you will work together to adjust your services as your needs evolve.
Institutional Respite:
Temporary or longer-term respite support in an ICF setting. Team RMS offers service provides support when a person’s regular caregiver, whether paid or unpaid, is unavailable or needs a break. Similar to other services, this option is available to those utilizing Individual Options, Level One, and SELF waivers.
The person’s service and support administrator (SSA) will evaluate the need for Residential Respite. If deemed necessary, the SSA will include Residential Respite in the individual’s service plan. There are two ways this service can be authorized.
The first option is Residential Respite authorized at a daily rate, which is limited to 90 calendar days per waiver span. The second option is Residential Respite provided at a 15-minute unit rate. This option is available specifically for those authorized for Ohio Shared Living, offering their primary caregiver a break. It is limited to a maximum of 12 hours per day and 52 hours per month.
Both agencies and independent providers with a Medicaid Provider Agreement and DODD certification can offer this service. Residential Respite at the daily rate can only be provided in an Intermediate Care Facility (ICF). However, both the daily rate and the 15-minute unit rate of Residential Respite can be offered in a non-ICF licensed Residential Facility, a person’s home, the home of an agency provider employee, the home of an independent provider, or another location chosen by the individual.
