PROGRAM COORDINATOR

POSITION OVERVIEW:  The position ensures that the appropriate, high quality services and supports are provided to the individuals served through effective care coordination and advocacy.  The Program Coordinator serves as an integral part of the Interdisciplinary Team (IDT), and ensures that an appropriate Individual Service Plan (ISP) is drafted and executed for the individuals on his/her caseload by completing the following: orienting and training staff on client goals , monitoring systems and documentation to ensure the ISP, as well as company procedures, are followed, and assisting with training and quality monitoring and improvement activities. 

POSITION REQUIRMENTS: A High School diploma and one (1) year experience providing direct care to individual’s with developmental disabilities.  A bachelor’s degree in social work, psychology, human services or other related field preferred.    Must have strong written and verbal communication skills and able to work successfully as part of a team.  Must have a current valid driver’s license  and automobile insurance meeting Ohio’s minimum standards.  Must be able to maintain CPR, First Aid, and medication administration certification.

Position available with RMS of Ohio, Central Region (Franklin and Fairfield Counties) 

SYSTEMS MANAGER

POSITION OVERVIEW: The Systems Manager is responsible for providing management oversight and staff supervision of supported living services.  The Systems Manager ensures that staff are trained and adequate in number to provide the services and supports prescribed in the individuals Individual Support Plan (ISP).  This position works collaboratively with the Interdisciplinary Team to ensure the delivery of high quality services.  This position requires a flexible schedule and the commitment to routinely provide direct support services.

POSITION REQUIREMENTS: A High School diploma or GED.  One (1) year experience in a supervisory role.  Previous experience providing supports to individuals with developmental disabilities strongly preferred.  Current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Ability to maintain CPR, First Aid, and medication administration certification. Previous experience with supporting individuals with developmental disabilities preferred.

SUPPORT ASSOCIATE

POSITION OVERVIEW: The Support Associate is responsible for providing supervision, support and assistance to consumers as prescribed within the Individual Service Plan/Individual Program Plan to increase independence and quality of life.

POSITION REQUIREMENTS: A High School diploma or GED. Current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Ability to maintain CPR, First Aid, and medication administration certification. Previous experience with supporting individuals with developmental disabilities preferred.

      Northeast Region (Cleveland-Cuyahoga County) FULL AND PART TIME--afternoons/evenings and weekends    
        Northwest Region (Toledo—Lucas County) FULL AND PART TIME—2nd shift, requires weekend availability 1st shift weekends available
          North Central Region (Lima—Allen & Putnam Counties)
            PART TIME--Putnam—3rd shift every other weekend PART TIME—Allen—2nd shift, must be able to work every other weekend

              Central Region (Columbus—Franklin County)--FULL AND PART TIME all shifts

                South Central Region (Dayton-Montgomery & Greene Counties)--FULL AND PART TIME all shifts 
                  Southwest Region (Cincinnati—Hamilton &Warren Counties) FULL TIME—2nd and 3rd shifts, requires weekend availability PART TIME—5a-9a; Monday-Friday
                     

                    DAY SUPPORT ASSOCIATE

                    POSITION OVERVIEW: The Day Support Associate is responsible for providing supervision, support and assistance to consumers participating in our Adult Day Services Program.  The Day Support Associate ensures the activities offered at the program are consistent with the individual’s preferences, as stated in the ISP.


                    POSITION REQUIREMENTS: A High School diploma or GED. Current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Ability to maintain CPR, First Aid, and medication administration certification. Previous experience with supporting individuals with developmental disabilities preferred.
