SERVICES COORDINATOR
POSITION
OVERVIEW: The Health Services
Coordinator manages the effective delivery of health services. This position provides management and
supervision for the House Nurse staff (3), and ensures that service delivery
meets the expectations outlined in the Individual Program Plan, the protocols
established by the Company, and the expectations identified by the Ohio
Department of Developmental Disabilities, Ohio Department of Job and Family
Services, and other stakeholders. The
Health Services Coordinator is also responsible for providing Delegated Nursing
and CPR/First Aide training to paraprofessional staff.
POSITION
REQUIREMENTS: Registered
Nurse with a current license in the state of Ohio. Five (5) years of nursing experience, three
(3) years management experience strongly preferred. Current knowledge of ICF/DD service
requirements and regulations preferred.
Must have strong written and verbal communication skills, and be a
highly effective problem solver.

HOUSE
NURSE
POSITION
OVERVIEW: This position oversees
all components of medical services delivered to individuals residing in a
ICF/DD group home. Responsibilities
include: coordination of medical services with outside providers (i.e.,
physicians, therapists, psychiatrist, dentists, etc), providing appropriate
information and follow up to the inter-disciplinary team, and providing ongoing
training and support to the home’s direct care staff. The Home Nurse provides after hours support
as necessary.
POSITION
REQUIREMENTS: Registered
Nurse or Licensed Practical Nurse with a
current license in the state of Ohio. At
least one (1) year of direct nursing experience. Current knowledge of ICF/DD service
requirements and regulations, as well as experience working with individuals
with developmental disabilities strongly preferred.

highly effective problem solver.

ASSOCIATE
POSITION
OVERVIEW: The Support Associate is responsible for providing
supervision, support and assistance to consumers as prescribed within the
Individual Service Plan/Individual Program Plan to increase independence and
quality of life.
POSITION REQUIREMENTS: A High School
diploma or GED. Current valid driver’s license and automobile insurance meeting
Ohio’s minimum standards. Ability to maintain CPR, First Aid, and medication
administration certification. Previous experience with supporting individuals
with developmental disabilities preferred.

SYSTEMS
MANAGER
POSITION
OVERVIEW: The Systems Manager is responsible for providing
management oversight and staff supervision of supported living services. The Systems Manager ensures that staff are
trained and adequate in number to provide the services and supports prescribed in
the individuals Individual Support Plan (ISP). This position works collaboratively with the
Interdisciplinary Team to ensure the delivery of high quality services. This position requires a flexible schedule
and the commitment to routinely provide direct support services.
POSITION REQUIREMENTS: A High School
diploma or GED. One (1) year experience
in a supervisory role. Previous
experience providing supports to individuals with developmental disabilities
strongly preferred. Current valid
