PROGRAM COORDINATOR

POSITION OVERVIEW: The Program Coordinator is responsible for care coordination for individuals served in our supported living program. The position ensures that the appropriate, high quality services and supports are provided to the individuals served through effective care coordination and advocacy.  This position works in conjunction with the Systems or site Managers, as well as the Interdisciplinary Team (IDT), and ensures that services are implemented as prescribed by completing the following: staff orientation and training, monitoring systems and documentation, coordinating the reporting of unusual incidents, coordinating client benefits, and other duties as assigned.

POSITION REQUIRMENTS:  At least (1) year experience providing direct care to individuals with developmental disabilities. A bachelor’s degree in social work, psychology, human services or other related field preferred.    Must have strong written and verbal communication skills and able to work successfully as part of a team.  Must have a current valid driver’s license  and automobile insurance meeting Ohio’s minimum standards.  Must be able to maintain CPR, First Aid, and medication administration certification.

Position available at RMS of Ohio, Northwest (Toledo/Lucas Co.) Region

PROGRAM COORDINATOR: ADULT DAY SERVICES

POSITION OVERVIEW: The Program Coordinator is responsible for care coordination for three adult day programs, located in Cincinnati. The position ensures that the appropriate, high quality services and supports are provided to the individuals served through effective care coordination and advocacy.  The Program Coordinator serves as an integral part of the Interdisciplinary Team (IDT), and ensures that an appropriate Individual Plan (IP) is drafted and executed for the individuals on his/her caseload by completing the following: orienting and training staff on resident goals and active treatment, monitoring systems and documentation to ensure the IP, as well as company procedures, are followed, and assisting with quality monitoring and improvement activities. 

POSITION REQUIRMENTS: A bachelor’s degree in social work, psychology, human services or other related field.  One (1) year experience providing direct care to individual’s with developmental disabilities.  Prior experience working in an Intermediate Care Facility for the Developmentally Disabled  or an Adult Day Program/Sheltered Workshop, and working knowledge of the rules and regulations managing ICF/DD services strongly preferred.  Must have strong written and verbal communication skills and able to work successfully as part of a team.  Must have a current valid driver’s license  and automobile insurance meeting Ohio’s minimum standards.  Must be able to maintain CPR, First Aid, and medication administration certification.

PROGRAM COORDINATOR (QIDP)

POSITION OVERVIEW: The Program Coordinator is responsible for care coordination for twenty-four residents residing in three ICF/DD group homes. The position ensures that the appropriate, high quality services and supports are provided to the individuals served through effective care coordination and advocacy.  The Program Coordinator serves as an integral part of the Interdisciplinary Team (IDT), and ensures that an appropriate Individual Plan (IP) is drafted and executed for the individuals on his/her caseload by completing the following: orienting and training staff on resident goals and active treatment, monitoring systems and documentation to ensure the IP, as well as company procedures, are followed, and assisting with quality monitoring and improvement activities.

 SUPPORT ASSOCIATE

POSITION OVERVIEW: The Support Associate is responsible for providing supervision, support and assistance to consumers as prescribed within the Individual Service Plan/Individual Program Plan to increase independence and quality of life.

POSITION REQUIREMENTS: A High School diploma or GED. Current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Ability to maintain CPR, First Aid, and medication administration certification. Previous experience with supporting individuals with developmental disabilities preferred.

DAY SUPPORT ASSOCIATE

POSITION OVERVIEW: The Day Support Associate is responsible for providing supervision, support and assistance to consumers participating in our Adult Day Services Program.  The Day Support Associate ensures the activities offered at the program are consistent with the individual’s preferences, as stated in the ISP.


POSITION REQUIREMENTS: A High School diploma or GED. Current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Ability to maintain CPR, First Aid, and medication administration certification. Previous experience with supporting individuals with developmental disabilities preferred.
