Position: Live-In Support Associate Reports to: Support Coordinator Description: The Live-In is a resident support associate, responsible for supervision, training and assistance to consumers as prescribed within the Individual Service Plan/Individual Program Plan to increase independence and quality of life. Responsibilities: 1. Provides service delivery in compliance with the ISP/IPP and company procedures, which are appropriately responsive to Consumers needs for growth, learning and independence. 2. Insures that the safety and health of consumers are maintained through continuous monitoring of medical status and their environment. 3. Works with Program Coordinator to insure twenty-four hour coverage, assists in identifying, scheduling and supervising relief staff. 4. Provides or facilitates transportation in accordance with Consumer's social, medical and general developmental needs. 5. Communicates and works effectively with Family, Guardian, Advocates, Case Managers, RMS peers and supervisors and the community. 6. Maintain consumer records and other documentation per established procedures. 7. Serves as leadership for a specific site in support of Program Coordinator. Evaluation Factors: 1. Consistent quality of service to consumers in compliance with plans documents and regulations. 2. Safety, cleanliness and personal growth of consumers and employee. 3. Internal and external relationships, cooperation and partnership with all concerned persons. 4. Timeliness, completeness and accuracy of oral and written communications. 5. Exercises maturity and good judgment in all matters, routine and unusual. Qualifications: 1. Ability to obtain CPR and First aid Certification. 2. Ability to work with persons with disabilities, exercise care and commitment. 3. Ability to operate independently and with little or no direct supervision 4. Ability to follow detailed plans accurately. 5. Ability to apply sound, mature decision making within prescribed limits. 6. Current valid driver's license and liability insurance required in most instances.
