Adult Day Services Coordinator
Reports to: Director of Operations or QAC
Description: The Adult Day Services Coordinator is responsible for
supervision, training and assistance to participants of RMS’ day
habilitation program. Services are to be provided within the
scope of the individual’s Individual Service Plan (ISP) with the
goal of increasing independence and/or improving the quality of
life. The Adult Day Services Coordinator maintains a liaison role
with the regional office and performs administrative duties.
Responsibilities:

Provides services in
accordance with the participant’s ISP and company procedures.

Insures that the
health and safety of consumers is maintained through continuous
monitoring of the environment.

Communicates and
works effectively with family members, significant others,
guardians, county board personnel and other RMS staff. Timely
and clear in all oral and written communications of both routine
and unusual matters.

Provides input
regarding consumer needs/accomplishments to peers, supervisors
and team members.

Maintains participant
records and other documentation per established procedures.
-
Coordinates the
functions of the support team as directed.
-
Opens and closes the
program during days of operation.
-
Initiates activities
in compliance with the ISP. Organizes participant schedules and
develops meaningful activities on an on-going basis.

Facilitates
transportation as needed. May be required to transport
participants on occasion.

Assures compliance
with all transportation rules and requirements.

Track services per
appropriate category (e.g. Supported Employment, enclave, etc.)

service in compliance with ISP’s, regulations and RMS
procedures.

cleanliness of the day hab environment.

relationships. Cooperation and partnership with all involved
persons.

judgment in all matters, routine and unusual.

Consistent
coordination of the program’s activities.
Qualifications:
-
Ability to
obtain/maintain CPR and First Aid Certification and to
successfully complete all other required training for the
position.

Ability to work
effectively with persons with disabilities.
-
Ability to work
effectively in a team environment.
-
Ability to follow
directions and complete required paperwork.
-
Successful background
check.
-
Current valid
driver’s license and liability insurance.
-
Successfully
completed pre-employment drug test (if providing non-medical
transportation)

Successfully
completed pre-employment physical (if providing non-medical
transportation)
