Position: Support Coordinator Reports to: Director of Operations Description: The Support Coordinator provides leadership and guidance to Support Associates and Team Coordinators in the delivery of services/supports in a supported living environment in fulfillment of the ISP. Responsibilities: Insures service delivery is in compliance with ISP and Company procedures. This includes monitoring individual cases, knowing individual consumers, and monitoring overall caseload. Trains, supervises and reviews Support Associates and Team Coordinators including setting of performance standards. Also participate in selection/hiring process. Communicates and maintains positive relationships with Family/Guardian, Advocates, Case Managers, RMS peers and supervisors, the counties and community. Monitors all relevant systems, including documentation sheets and procedures, medications, finances, and physical environment at point of service. Participates in and provides cross training as requested.
Evaluation Factors: Consistent quality of service to Consumers in compliance with plan documents. Leadership, supervisory ability, development and maintaining of talented workforce. Maintains professional manner at all times. Internal and external relationships, cooperation, partnership as evidenced by positive feedback from all concerned. Cost control. Treats all employees consistently and fairly. Understands and adheres to established policies and procedures. Incident report review - quantity/severity/trends. Maturity, organizational skills, proactive management.
Qualifications: Undergraduate degree in education, rehabilitation, psychology, human development, or a related field, or be a registered nurse is preferred. One year working with persons with mental retardation/developmental disabilities. Current CPR/First Aid certificate. Ability to lead, supervise, and operate in a team environment. Ability to apply good, mature decision-making within prescribed limits. Current valid driver's license and liability insurance. Ability to obtain and maintain Delegated Nursing/Med. Admin. certification.
