SUPPORT ASSOCIATE

Position:          Support Associate

Reports to:     Support Coordinator/Program Coordinator or Systems Manager

 

Description:  The Support Associate is responsible for providing supervision, training and assistance to consumers as prescribed within the Individual Service Plan/Individual Program Plan to increase independence and quality of life.

 

Responsibilities:

1.      Provides service delivery in compliance with the ISP/IPP and company procedures, which are appropriately responsive to Consumers needs for growth, learning and independence.

2.      Insures that the safety and health of consumers are maintained through continuous monitoring of medical status and their environment.

3.      Communicates and works effectively with Family, Guardian, Advocates, Case Managers, RMS peers and supervisors and the community.

4.      Timely and clear in all oral and written communications of both routine and unusual matters.

5.      May provide or facilitate transportation in accordance with consumer's social, medical and general developmental needs.

6.      Provide input regarding consumer needs/accomplishments to the immediate supervisor and team.

7.      Maintain consumer records and other documentation per established procedures.

8.      Maintains and monitors coaches assignments (where applicable) at the discretion of the supervisor.

3.      Internal and external relationships, cooperation and partnership with all concerned persons.

5.      Exercises maturity and good judgment in all matters, routine and unusual.

 

Qualifications:

1.      Ability to obtain CPR and First aid Certification.

2.      Ability to work with persons with disabilities, exercise care and commitment.

3.      Ability to operate in a team environment, clear in oral and written communications.

4.      Ability to follow detailed plans accurately.

5.      Ability to apply sound, mature decision making within prescribed limits.

6.      Current valid driver's license preferred and liability insurance.
