POSITION DESCRIPTION

 
Position: Nurse - RN/LPN
Reports to: Health Services Coordinator
Description: The nurse will be responsible for the implementation of health services for consumers in RMS homes.

        Responsibilities

Coordinate the provision of a total program of health services for consumers in assigned homes.

 

Assist in training of staff which may include medication administration and observation, infection control, 0511k and how to meet the health service needs of the consumers.

 

Identify the signs and symptoms of illness in the consumers, including use of a quarterly physical assessment, and report medical issues as required.

 

Administer medications and treatments as needed and complete all medical documentation as required by State and Federal regulations.

 

Develop professional relationships with family members, medical professionals, outside agencies and the OT in support of the medical needs of the consumer.
 

Evaluation Factors:

All medical information concerning assigned consumers is timely and

complete.

 

Quality of health services delivered to the consumers.

 

Responsiveness to consumers medical needs.

 

Medical tracking system is current and complete.
 

Qualifications:

LPN or RN with current State license.

Experience with persons with Intellectual and other Developmental Disabilities preferred.

Ability to work as a team member in cooperative relationships with other professionals,

consumers and staff
