The Assessment

Before the ISP meeting, your Service Coordinator will review your rights with you and ask you to sign a paper saying that you understand your rights.

Your Service Coordinator will meet with you and anyone else you would like to be there to create your ISP.

Your Service Coordinator will ask you questions about your life and the areas below:

  1. CHOICES AND OPTIONS

  2. HEALTH

  3. PERSONAL INCOME

  4. PERSONAL SATISFACTION

  5. SAFETY

  6. HOUSING

  7. COMMUNITY MEMBERSHIP

Your Service Coordinator and anyone else you have invited will help you decide if you need or want help in any of these areas.

The ISP meeting is an opportunity for you to develop a plan that will enable you to live a happy and independent life.

Your ISP meeting and your plan should:

  • Focus on you and help you participate in deciding what you want to do and what kind of help you will need.

  • Help you become more independent and learn how to make good choices and decisions.

  • Help you maintain friendships and be a part of your community.

  • Help you reach your goals in life.

Once you decide what types of help you will receive your Service Coordinator will create your Individual Service Plan.

The information contained in your ISP should include:

  • Your name.

  • Where the money to pay for your services will come from.

  • The date your Individual Service Plan was created.

  • How long your plan is good for. This is called the "span period".

  • The areas of your life where you will receive help.

  • The type of help you will receive.

  • Who will provide your help in each area.

  • How often and how long you will receive help in each area.

Once your ISP has been created you and your provider should receive a packet of information including a copy of your new ISP.
